I hereby acknowledge that Neighbourhood Watch Victoria may request me to provide a police check to determine whether I am suitable to become a member of Neighbourhood Watch Victoria. I understand that the results of my CrimCheck may be provided to Victoria Police in the unlikely event that they request it.

I understand that Neighbourhood Watch Victoria grants all authorisation or accreditations for any individual to participate in or represent the Neighbourhood Watch Program in Victoria, and that Neighbourhood Watch Victoria reserves the right to withdraw such authorisation or accreditation at any time.
